At University of Vermont Health, high-quality care isn’t just available in hospitals and doctor's offices. Home health care allows you to receive personalized treatment and support in the comfort of your own home.

We offer comprehensive home health services for individuals and families at every age and stage of life. Whether you need rehabilitation after illness or injury, monitoring for a chronic condition, or support for your family, our compassionate home health professionals are there when you need them most.

Care at Home Services

Some conditions can be monitored and treated from home, meaning you don’t have to travel to get the care you need. Home health care services are available to eligible patients across Vermont and northern New York, including:

Breastfeeding support includes personalized education, assistance and encouragement to help you meet your breastfeeding goals.

Cardiovascular home monitoring record your heart rhythm remotely through small, wearable devices such as the Holter monitor or a transtelephonic monitor.
Treatment
Home infusion pharmacy services support patients taking medication delivered through a catheter or needle (infusion therapy) who are able to get this treatment at home.

Sleep disorder home monitoring includes services such as: Portable devices for Home Sleep Apnea Testing (HSAT) and home actigraphy monitoring.
Treatment
Digital health, known as telemedicine or telehealth, is a convenient way for you to access routine visits, follow-up appointments, post-operation care, medication review and more.

Additional Care at Home Services in Vermont

UVM Health offers additional home health services to eligible residents of Vermont's Chittenden and Grand Isle Counties. Through a wide range of programs, we provide care, guidance and support for individuals and families at every age and stage of life.

If you live outside of Chittenden and Grand Isle Counties, you can find additional home health services at the VNAs of Vermont or through NY Connects.

Expert, compassionate hospice and palliative care for people at the end of life and those living with life-altering illnesses.

Home-based care and support for children and families, including wellness and prevention services, nursing care and pediatric rehabilitation therapy.

Comprehensive home health care for people recovering from illness, injury or surgery, including nursing care, physical and occupational therapy, and personal care.

Support for families caring for older adults and people with disabilities and chronic medical conditions, including respite services and long-term care.
