Whether you need rehabilitation after illness or injury, monitoring for a chronic condition, or support for your family, our compassionate home health professionals are there when you need them most.

Some conditions can be monitored and treated from home, meaning you don’t have to travel to get the care you need. Home health care services are available to eligible patients across Vermont and northern New York, including:

Additional Care at Home Services in Vermont
UVM Health offers additional home health services to eligible residents of Vermont's Chittenden and Grand Isle Counties. Through a wide range of programs, we provide care, guidance and support for individuals and families at every age and stage of life.
If you live outside of Chittenden and Grand Isle Counties, you can find additional home health services at the VNAs of Vermont or through NY Connects.
