At University of Vermont Health, your health journey begins with primary care. Primary means “first,” and having a primary care provider is an essential first step to better health.

We help you and your family achieve health goals through preventive care, wellness education, family planning and pregnancy care, and disease management. With a range of experts and convenient clinic locations throughout Vermont and northern New York, you’re sure to find the trusted medical care you and your family need. Our providers partner with you, listen to your concerns and get to know you before developing care plans that meet your unique health needs.

  • Experienced providers: Our team includes highly skilled primary care providers with distinct specializations, including reproductive and pregnancy care. When appropriate, we work with disease specialists throughout UVM Health to provide more advanced care.
  • Dedicated care team: Primary care services include comprehensive support from a dedicated Care Management team. We can help make care more accessible through assistance with insurance and financial issues, transportation to medical appointments and other services.
  • Care for mind and body: Your mental health is as important as your physical health. Our primary care can connect you with psychiatrists, psychologists, social workers and help for substance use disorders. Sometimes these services are embedded within the practice.
  • Patient-Centered Medical Homes: All of our primary care practices are certified Patient-Centered Medical Homes. This designation from the National Committee for Quality Assurance (NCQA) reflects our ongoing commitment to providing high-quality, team-based care to all patients.

Adult primary care provides complete medical services for people 18 and older.
