On any given day, we serve more than 1,000 neighbors throughout Chittenden and Grand Isle counties. Through a wide range of programs, we provide care, guidance and support for individuals and families at every age and stage of life.

Our Story

Rooted in over 100 years of services as the founders of the Visiting Nurses Association in Vermont, we’ve grown to meet the changing needs of our community. Today, Home Health & Hospice supports individuals and families through every stage of life, from pregnancy to end-of-life.
